Consent Form




Our Purpose: To move people from crisis to spiritual health using crisis counseling and spiritual direction with the goal of connecting them to a place of service and/or a community of spiritual friends at CS.


Biblical Basis: We believe that the Bible and the Holy Spirit give guidance to solve life’s problems. Our counselors are not trained or licensed as psychotherapists or mental health professionals.


Regarding Professional Advice: Some of our Lay Counselors may work in professional fields outside the church. When serving as Lay Counselor within our church, however, they do not provide professional advice and services as they do when they are functioning in their professional capacities. Therefore, if you have significant legal, financial, medical or other technical questions, you should seek advice from an independent professional. Our Lay Counselors and trained counselors are open to cooperating with such professionals and helping you to consider their counsel in the light of relevant scriptural principles.


Confidentiality: Confidentiality is an important aspect of our counseling process and we will hold all information confidential. There are some situations, however, when it may be necessary for a counselor to share certain information with a pastor on the staff. These are: 1.) When the Lay Counselor is uncertain of how to address a problem and needs to seek advice from a pastor; 2.) When someone is harmful to oneself or others; 3.) When an individual’s behavior would directly impact a ministry in the church. Appropriate authorities may need to be made aware of the situation. Please be assured that our Pastoral and Lay Counselors strongly prefer not to disclose personal information to others, and they will make every effort to help you find ways to resolve a problem as privately as possible.


Resolution of Conflicts: Sometimes a conflict may develop between a counselor and counselee. To make sure that conflicts are resolved in a biblical manner, we require all counselors to agree to settle by biblical mediation.


Agreement: Having clearly stated the principles and policies of our Pastoral and Lay Counseling ministry, we welcome the opportunity to minister to you and pray with you.
